FAQ's - New Appointments

Please call our Toms River office at 732-736-7007 or our Lakewood office at 732-886-5872 to schedule your first appointment.

To assure the best care for you, we ask that you bring the following with you to your first appointment:

1. A valid prescription for physical therapy
2. A referral, if appropriate
3. Insurance card
4. Appropriate clothing based on the area being treated:

• BACK/NECK – comfortable clothing like sweats or shorts, t-shirt and sneakers
• SHOULDER/ELBOW – short sleeve/sleeveless shirt, sports bra
• KNEE/HIP – shorts, pants that can be rolled up to mid-thigh, t-shirt, sneakers
• ANKLE – shorts or loose fitting pants that can be rolled up to the knee, sneakers
• ANODYNE TREATMENT – shorts, loose fitting pants, sneakers
